# Prepare the evidence before a consult or multidisciplinary meeting

Before a cancer meeting or a clinic, gather results, reports and outside records into one brief. Every fact shows its source and date. The systems by layer, the work today, the same work with one agent, and who signs.

A patient is listed for a weekly multidisciplinary meeting (MDT), such as a lung cancer meeting. Or the patient is on tomorrow's clinic list. Before the meeting, someone must gather results, imaging and pathology reports, and outside records. Today an MDT coordinator and a registrar do this by copy and paste across many screens. The result should be one brief where every fact shows its source and date. With an agent, the gathering and drafting move to the agent. A clinician signs the brief. Specialists still read the images and slides, and the MDT still decides.

## The systems involved [#the-systems-involved]

MDT preparation touches these layers. The [healthcare page](/use-cases/healthcare) shows the full hospital estate.

Clinic preparation is the same pattern, smaller. A clerk checks the results are back. A nurse prints them. A registrar reads them before the clinic.

<TeachingIllustration src="/figures/use-cases/consult-as-is.svg" mobileSrc="/figures/use-cases/consult-as-is-mobile.svg" title="Preparing for a multidisciplinary meeting today" alt="Swimlanes for outside providers, the MDT coordinator, the registrar, radiology and pathology, the MDT and chair, and the systems. Step 1: the coordinator lists the patients in a spreadsheet. Step 2: the coordinator gathers results from several screens. Step 3: the radiologist sets up images. Step 4: the pathologist reviews slides. Step 5: the coordinator chases outside reports, which often arrive late. Step 6: the registrar writes the case summary by copy and paste. Step 7: the MDT meets and discusses. Step 8: the chair signs the outcome, and it goes to the EMR and the GP. Step 9: the data warehouse reports activity. A blue box under each step names the system it uses." caption="The case summary is copied from many screens, with no source or date on each fact." />

**Handoffs.** The referring team hands to the MDT coordinator. The coordinator asks radiology, pathology and outside providers for what is needed. The MDT hands its outcome to the treating team and the GP.

**Where reporting comes in.** Cancer services count MDT activity. They track time to treatment against the Optimal Care Pathways. Some outcomes must be notified to the cancer registry.

**Volumes and cycle time.** An MDT may discuss 10 to 30 patients a session (typical, unsourced). Preparation takes several hours of coordinator and registrar time per meeting (typical, unsourced). No Australian or New Zealand figure was found for MDT preparation time.

**Pain points.**

* Outside reports are missing on the day.
* Data is stale, because the latest result came after the summary was written.
* Images are not available, so the case is deferred.
* Summaries show no source or date.
* The same patient comes back next week because something was missing.
* No record exists of what the MDT actually saw.

Since 1 July 2026, outside pathology and imaging reports go to My Health Record by default (Australian Digital Health Agency). Outside reports are now easier to find, where the service's policy allows their use.

## How agents run it [#how-agents-run-it]

One agent runs fixed steps. It reads and drafts. It gives no treatment advice.

**What stays with a person, and why.**

* Reading images and slides. That is specialist clinical judgement.
* Staging and the treatment recommendation. That is a clinical decision.
* Signing the outcome. It is the legal record.
* Using outside records beyond the service's policy. That is a matter of consent and privacy.

**Where the evidence is kept.** The source list is saved with the brief. It shows exactly what the MDT saw. The EMR audit log records each read under the agent's identity, on behalf of the named clinician. Each version of the brief is kept. Agent <Term k="trace">traces</Term> sit outside the agent.

**What changes in the systems.**

* The MDT module gets "brief drafted" and "brief reviewed" statuses, and a place for the source list.
* An agent identity with read access limited to the patients on the list.
* An event for a new result on a listed patient, so the brief stays current.
* A written policy on using My Health Record documents for MDT preparation.

**What does not change.** PACS and the LIS stay the sources. The MDT makes the recommendation. The signed outcome stays the legal record. Registry reporting stays as it is.

No Australian or New Zealand public hospital was found using an agent to prepare MDT briefs.

## Where this sits [#where-this-sits]

The [four questions](/architecture/agent-types) place this agent.

* **Role.** It creates: it writes a brief. The domain is health records.
* **Permissions.** These are what its own credentials let it do. It reads records for listed patients only, on behalf of the named MDT lead. It writes draft briefs, draft notes and follow-up tasks. It drafts outside requests that the coordinator approves. It has no emergency override access.
* **Autonomy.** It is designed and permitted for delegated tasks (A2) on the [autonomy contract](/architecture/autonomy-contract). Every brief is reviewed and signed by a clinician. Anything that looks like advice drops to assisted (A1), where clinicians decide.
* **Impact.** It touches none of the four [deterministic zones](/architecture/deterministic-zones). The rule for decisions about people applies to staging and treatment. The MDT makes them, and the chair signs. A wrong or stale fact in a signed brief could shape care, so every fact carries its source.

It is [Tier 3 (strict)](/architecture/risk-profile), because it reaches regulated health data. Outside reports are untrusted input. It reaches health data, and it sends requests to outside providers. That is all three parts of the <Term k="rule of two">Rule of Two</Term>. So it stays at A2, and the coordinator approves every outgoing message.

Its learning level is fixed (L0). It follows a fixed template and does not learn from cases. A recommendation feature would be a separate tool, with its own TGA check, added only through the [agent lifecycle](/architecture/agent-lifecycle).

It has its own identity (ID2). The [delegation chain](/architecture/delegation-chain) records the named MDT lead it acts for. [Policy is enforced outside the model](/architecture/enforcement): the EMR grants read access only for listed patients. Of the [seven capabilities](/architecture/system-view), [knowledge and memory](/memory) holds the source list, and observability and audit trail keeps every brief version.

## Measure the result [#measure-the-result]

Compare briefs from past meetings with the summaries people wrote. Measure these:

* Missing items on the day, and cases deferred because of them.
* Stale items caught by the refresh step.
* Corrections the clinician makes before signing.
* Facts in the brief with no source. The target is none.
* Coordinator and registrar hours per meeting.

## Start with a trial [#start-with-a-trial]

Pick one MDT, such as the lung cancer meeting. Draft briefs for past meetings and compare them with what was presented. Then run it live, with every brief signed before it is shown. Register the agent with a named sponsor and owner, and agree the My Health Record policy first.

## Evidence and limits [#evidence-and-limits]

No Australian or New Zealand figure was found for MDT preparation time. The volumes here are typical, unsourced.

The closest named case, Oxford's TrustedMDT, is an approved pilot. Oxford says it needs more evaluation before clinical use. Microsoft's orchestrator is at research stage for tumour boards.

The design gives no treatment advice. A tool that suggests a diagnosis or treatment may need TGA listing. Check the TGA position before adding any such feature.

## Next step [#next-step]

Hand the signed outcome to [care follow-up](/use-cases/care-follow-up). Or go back to the [hospital estate](/use-cases/healthcare).
